Currently I am getting 45 miles per US gallon.
Is that good?

I have a 2012 base model.

Is there significant difference in mpg between eco and normal mode?
Does it affect mpg if you're constantly changing modes?

How do you all check on how much mpg you got on normal mode compared to eco mode?

This was a very common question early on in the car's life so you can get a lot more detailed discussion by searching. However, I'll give you the quick answer to say that the different drive modes adjust the throttle and steering mapping. It doesn't, by itself, change the mpg of the car. That is, you could drive in sport mode very conservatively and get better mpg than someone who is in eco and driving very aggresively. Personally I put it in sport as part of my start up routine and have never looked back. I know a lot of others on here do too. The car is so slow as it is, I can't imagine driving it in any other "slower" mode than sport. So get the most responsiveness out of it that you can and enjoy the great mpg.
