yep, the early CT200s had head gasket problems exactly as you're describing. Coolant leaks in and fouls up the plugs and throws codes and causes shuddering.

They actually don’t have any engine problems. There were no specific changes through the engine series.
I’ve been working at Lexus for 15+ years in the workshop.
